The fatality involves a patient who travelled from the Democratic Republic of Congo through Uganda before arriving in Nairobi, exposing vulnerabilities in regional border screening and heightening fears that the epidemic has reached further than previously documented.

The strain responsible for this case is the Bundibugyo virus, commonly known as BDBV, which was first detected in neighbouring Uganda in 2007. The broader epidemic, which began earlier this year in the DRC, has severely strained local healthcare infrastructure. According to the World Health Organization, the ongoing outbreak in the DRC has claimed at least 4,148 lives out of 8,300 reported cases. The virus also crossed the border into Uganda earlier this year, resulting in approximately 20 cases before that country was declared Ebola-free in July.

However, the recent death in Kenya has cast doubt on Uganda’s disease-free status and raised urgent questions regarding the rigidity of Kenya’s screening protocols for incoming travellers. Health officials and epidemiologists are now grappling with fears that the virus may have circulated more widely along travel corridors than current surveillance indicates.

In response to the development, the World Health Organization announced that the Kenyan government is actively enhancing its disease surveillance systems, implementing more targeted and rigorous screening procedures at high-risk points of entry. Despite these containment measures, public health experts warn that the sheer scale of the epidemic presents an unprecedented challenge.

Wolfgang Preiser, a professor and head of medical virology at Stellenbosch University in South Africa, noted that while concerted efforts are underway to bring the outbreak under control, the sheer volume of cases and the velocity of transmission are overwhelming regional medical systems.

“I am not surprised that cases have reached other provinces in DRC and neighbouring countries. I expect that this will continue to happen until such time that the trajectory has been reversed and case numbers are in decline,” Preiser said.

What is Ebola?

Ebola is a serious, potentially fatal viral infection transmitted through contact with the bodily fluids of an infected person or wild animal, including fluids lingering on surfaces. Historically, individuals have also contracted the disease through the consumption of contaminated bushmeat.

The African continent has experienced numerous outbreaks across its central, western, and eastern regions. A notable West African outbreak between 2014 and 2016 resulted in at least 11,300 deaths from 28,600 reported cases, a crisis that significantly accelerated the development and deployment of vaccines. That epidemic was driven by a different strain of the virus, known as Zaire, for which effective vaccines are now available.

In contrast, the current outbreak is fuelled by the Bundibugyo virus strain. Epidemiologists believe this particular wave was triggered by a novel animal-to-human transmission event, followed by sustained human-to-human transmission, rather than by genetic variants descended from earlier outbreaks. Crucially, there is currently no approved vaccine for the Bundibugyo strain.

Symptoms of the virus typically manifest between two and 21 days following exposure, beginning abruptly with flu-like indicators such as a high fever, fatigue, and severe headaches. As the infection progresses, it can cause internal and external bleeding, severe impairment of liver and kidney function, and ultimately multi-organ failure.

How Did This Case of Ebola Reach Kenya?

Kenyan Health Minister Aden Duale provided details regarding the patient, who has not been publicly named. The individual had been living in the DRC for several years, where they first fell ill a month ago and received initial local treatment.

On October 2, the patient initiated a journey by road from Beni in the DRC to the Ugandan capital of Kampala, before boarding a flight to Nairobi and arriving the following day. Upon landing in Kenya, the traveller was transported by a relative and a friend to a hospital, where medical staff promptly isolated him. Subsequent laboratory tests confirmed he was infected with the virus. Despite receiving medical care, the patient died on Monday and was buried on Tuesday in accordance with national Ebola management protocols.

Public health authorities in Kenya have since identified 28 potential contacts, including family members and the healthcare workers who treated the patient. Additionally, officials are actively tracking 23 passengers and four crew members from the same flight. The WHO reported that arrangements are underway to provide appropriate medical follow-up and quarantine for individuals assessed to be at risk.

Jean Bisimwa Nachenga, a professor of infectious diseases at Stellenbosch University, emphasized that infectious pathogens operate independently of geopolitical boundaries.

“Population mobility, displacement, cross-border trade, and fragile healthcare systems make containment particularly challenging. In eastern DRC, ongoing insecurity further complicates surveillance, contact tracing, and access to affected communities,” Nachenga said. “Regional cooperation is therefore essential.”

How Did a Sick Patient Get Past Screening in Both Uganda and Kenya?

Travellers moving to, from, or through Uganda and Kenya are routinely subjected to multiple airport temperature checks and must complete at least two digital screening forms designed to identify potential exposure to Ebola in the DRC. Despite these layers of security, the infected individual managed to bypass detection at multiple checkpoints.

Ugandan government spokesperson Alan Kasujja rejected suggestions that Kampala bore responsibility for the traveller contracting or moving the virus, writing on social media that Uganda does not have active cases of the disease. In an official statement, the Ugandan Ministry of Health maintained that the patient registered a normal temperature when screened prior to departure at Entebbe International Airport.

Meanwhile, Kenyan authorities suggested the possibility that the individual may have ingested medication to mask their symptoms during thermal screening at Nairobi’s airport, though investigations remain ongoing.

Richard Mugahi, a senior Ugandan health official, told reporters that authorities are working to retrieve the digital health declaration forms completed by the traveller at Entebbe airport to verify what information was provided regarding recent health issues and travel history in the DRC. Officials are also reviewing airport security footage to identify the driver who transported the passenger to the terminal, facilitating broader contact tracing efforts in Uganda.

Preiser noted that despite this breach, the broader diagnostic and containment infrastructure functioned rapidly once the patient sought medical attention within Kenya. He drew parallels to historical incidents, such as an occurrence during the West African outbreak when a British nurse fell ill during her return journey to the UK, passed initial medical checks at Heathrow Airport, and was only diagnosed after reaching her final destination. Such events, he observed, demonstrate that even cooperative travellers and robust screening systems can occasionally experience gaps.

Where Else Has Ebola Spread?

Since its emergence in the northeastern Ituri province of the DRC earlier this year, the outbreak has spread to seven provinces across the country’s northern and eastern territories. The crisis was officially declared an epidemic in May.

Containment efforts have been severely hampered by weak local infrastructure, the geographic remoteness of eastern DRC, and persistent armed conflict near the borders with South Sudan, Uganda, and Rwanda. The response has faced additional complications, including strikes by unpaid healthcare workers, widespread misinformation, and cultural traditions such as open-casket family burials practiced earlier in the outbreak, which heightened transmission risks.

Further compounding the crisis, the United Nations reported that an Ebola treatment transit camp on the outskirts of Bunia, the capital of Ituri province, was burned down by soldiers searching for weapons. The incident forced approximately 19,000 displaced individuals to flee the facility.

The disease has also demonstrated an ability to cross international borders, evidenced by the 20 cases recorded and treated in Uganda earlier this year before that country achieved Ebola-free status in July. Controlling the virus within the DRC has grown increasingly difficult. Medical humanitarian organisation Doctors Without Borders warned of an alarming surge of cases in eastern North Kivu province, which shares a border with Uganda and currently accounts for 40 percent of all new infections.

Stephanie Hoffmann, a coordinator at the organisation’s treatment centre in Butembo, described the situation as fighting a megafire, noting that multiple outbreaks are developing simultaneously across different locations with varying intensities. North Kivu’s urban centre and surrounding areas house roughly two million residents but rely on only a limited number of treatment facilities, necessitating frequent patient transfers that elevate transmission risks.

What Happens Now?

The World Health Organization is actively collaborating with Kenyan authorities to execute contact tracing and reinforce health screenings for incoming travellers. Mohamed Janabi, the WHO regional director for Africa, stressed the importance of proactive health emergency preparedness.

“Kenya has put important outbreak control measures in place. The priority now is to move swiftly to detect any further cases before the virus has an opportunity to spread. We’re supporting the ongoing efforts to strengthen the response, and with rapid and coordinated action, we can prevent the virus from gaining a foothold and stop a potential larger outbreak,” Janabi said.

As part of these support measures, the agency has delivered approximately 1,000 Ebola diagnostic tests and 1,000 personal protective equipment kits to high-risk counties across Kenya.

Earlier in the legislative cycle, the Kenyan government granted permission to the United States to establish an Ebola quarantine facility at the Laikipia airbase, located roughly 120 miles from Nairobi, intended to treat infected American citizens travelling from African nations prior to their repatriation. That initiative sparked local protests over transmission fears, leading Kenyan courts to halt the project.

Reflecting on the broader strategic outlook, Nachenga argued that enhancing border screenings alone is insufficient without a simultaneous reinforcement of the entire public health infrastructure.

“This means training frontline healthcare workers, ensuring rapid laboratory diagnosis, promptly isolating suspected cases, and tracing and monitoring contacts. Kenya’s ability to identify this case provides an important opportunity to strengthen preparedness,” he concluded.

NAIROBI — Kenya has reported its first death from a deadly and fast-spreading strain of the Ebola virus, marking a critical new phase in an outbreak that has already devastated parts of Central Africa.
